The Medical Treatment of Osteoporosis in Elderly Population: Focus on SERM (Selective Estrogen Receptor Modulator)

Abstract
Osteoporosis is the most common metabolic disease in elderly population that result in fragility fractures. So osteoporosis in elderly population should be considered as a serious public health concern. Despite this consideration, osteoporosis in elderly population is still underdiagnosed and undertreated. The evaluation of elderly population for osteoporosis requires medical history, physical examination and diagnostic tests. Management focuses first on nonphamacologic strategy (such as balanced diet, adequate exercise, adequate calcium and vitamin D intake, fall prevention) If phamacologic therapy is indicated, there are a variety of phamacological agents that have been government approved options for osteoporosis including bisphosphonate, selective estrogen receptor modulator, hormone replacement therapy, parathyroid hormone, strontium and calcitonin. As well, In elderly population decreased renal function, delayed excretion of drugs, having other disease, and taking other drugs may occur, when prescribing osteoporotic medication for them, precise caution should be needed. In this article, Although numerous pharmacologic therapies are currently approved for the prevention and/or treatment of osteoporosis, I will provide the medical strategic options of osteoporotic treatment focused on SERM (Selective Estrogen Receptor Modulator) in elderly population.
